Membership Term and Fees
This Membership Package is NOT an insurance product. Services are provided exclusively via telemedicine within the State of Arizona and exclude all in-person procedures and facility charges.

Enrollment Fee: $75.00 (One-Time, Non-Refundable). Covers onboarding and medical record setup
Monthly Membership Fee: Auto-debited monthly on the 1st of each month.
Services Included
- Unlimited virtual consultations as medically necessary.
- Same-day or next-day appointments for urgent or acute needs.
- All follow-up visits (lab review, medication adjustments) at no additional cost.
- Direct secure messaging access.
- Weight management included at no extra monthly cost (limited-time offer).
Services and Costs NOT Included (Exclusions)
- No in-person care including physical exams, vaccines, sutures, or procedures.
- Member is responsible for all third-party costs (labs, imaging, specialist visits). Insurance will be used whenever possible.
Term, Cancellation, and Re-Enrollment
- 30-day written notice required for cancellation.
- If cancelled within the first 90 days, a fee equal to one month’s membership will apply.
- Patients re-enrolling within 12 months must pay a Re-Enrollment Fee.
